I did not receive the refund check that DRS said was sent to me
  Question

Q. DRS said they mailed a refund check to me (or my business) but I never received the check.  What should I do?

  Answer

A. If you have not received the check after approximately 2 weeks from the date that DRS mailed it to you, complete Form CT-3911, Taxpayer Statement Regarding Refund, and mail it to the address on the form.  This form should be used by individuals and by businesses to report that a check from DRS was not received.

DRS will research the matter and contact you.  In most cases, DRS will find that the check was not cashed and will place a stop payment on the check and issue a new check.
